1. You must arrive 10min before your appt and CALL 773-397-0448 to be met at door, sign form and have temperature taken. Your appt will be cancelled if you temp is 100.3+

2. Absolutely no late arrivals. No grace period.

3. Come alone. No friends, family, children or pets. 

4. No restroom. 

5. Must wear a mask to enter. 

6. No luggage or large personal belongings. Bring bare minimum. 

7. Clean shaved and clean skin. We will absolutely not be carrying razors.

8. Cannot laser if you have been on antibiotics in the last 7 days. 

9. Please do not come if you are not feeling well or have had exposure to anyone that has exposure to/tested positive for COVID-19 in the last 14 days.
